Scheduling

To schedule an appointment, please contact our office at (270) 761-5804.  When you call our office to schedule, we will collect some basic information about you. This information is then passed to the clinical team to determine the best provider for your needs.  You will then be assigned a clinician and contacted by a member of our team to set up your initial appointment.  This process may take up to a week to complete.  Most appointments are scheduled within 1-2 weeks from the time of calling.  If you are calling with an emergency, please indicate such and you will be referred appropriately.  Please contact the 24-hour crisis line at (270) 251-2924 if needed.

Payment

Services are provided on a fee for service basis.  Check, Cash, Visa, and MasterCard are accepted for payment of services at the time of service.

Cancellation Policy

Bridges Family Center, LLC understands that you may occasionally need to cancel or reschedule an appointment. Scheduled appointment times are reserved especially for you. If an appointment is missed, or cancelled with less than 24 hours’ notice, you (not your insurance company) will be charged the full fee of the session.

Intake Paperwork

Please print and complete this packet to the best of your knowledge prior to your first appointment.  If you have questions, you can leave portions blank and they can be completed at your first appointment.

Insurance:  We are in network with most private insurance companies.  However, we are not participating providers with Medicaid, Medicare, or plans associated with the Affordable Health Care Act.  Services may be covered fully or partially by your health insurance or EAP plan.  Please check to see if your policy has mental health coverage and if pre-authorization is required.  If you need assistance in determining your benefits, Bridges Family Center, LLC may be able to assist you.  If you opt to use your health insurance plan to pay for services, you will be responsible to pay for any co-pays, deductibles, coinsurances, and cost share amounts as indicated by your insurance company.

Initial Intake Fee: The initial intake fee is $150.00 for individual or family sessions. If you have insurance we accept, you may opt to use your insurance to cover this fee. The initial intake fee for couples is $200.00. These fees are due at the time of the initial intake appointment.

Group sessions: All group sessions are $30.00 per session which must be paid at the beginning of each group session.  Groups are not billed to insurance.

Counseling Sessions: The standard fee for all sessions is $100.00. This fee is due at the time of the appointment. If you have insurance we accept, you may opt to use your insurance to cover this fee.
